Business Registration Form

Thank you for your interest in being added to our directory of potential businesses for consideration.  

  1. Fill out the registration form below for your company. Please submit only one form. Submitting more than one form will not increase your likelihood for opportunities. If any registration information changes after your initial submittal, please send updates via email to supplierdiversity@wustl.edu.
  2. After you submit the form, you will receive a confirmation email with a link to schedule a follow up phone call.
  3. Additionally, you may upload electronic business documents to our box portal (optional). These might include your W-9 form, ownership certification(s), and other supporting documents you wish to share. File names need to include your business name, last name and doc description (e.g., company-lastname-description.doc). PDF or DOC format.

Reminder: While inclusion in our directory increases your visibility to the university, participation is not required prior to engaging in the WashU procurement process.
